What happens after you submit the form?
Your submission is reviewed by appropriate school staff (such as administration, student support, or counseling personnel). A team member will follow up as needed, typically within 1–3 school days, to discuss next steps or gather additional information. Information shared through this form is treated with care and respect and is only shared with staff members who need it to support your child.
